On "The Adventures of Rocky and Bullwinkle" for the NES, Boris' face on the game over screen scared the crap out of me. I was like 5. I was really bad at the game too so it would haunt me every ten minutes or so.

I dunno, seeing as I haven't played a lot of video games.  Doom freaked me out.  Myst isn't really scary, but it gives you a paranoid feeling.  Most ambient adventure games are like that.
